I have both of these problems and enough is enough. What do I need to do to fix this so I have good posture?

Cyan, while I suspect that weak back muscles (including the traps) may play a role, I suspect that an awful lot of posture is learned. If you want to try the weight training approach, I would suggest doing different types of shrugs, not just the standing ones that everyone knows about (get Kelso's Shrug Book to learn about incline shrugs, etc.). That having been said, I suspect most people will need to take some fancy physical re-education course like The Alexander Technique.
Note, I'm not promoting the book or the Alex Technique. I'm just giving you some options. I'm no expert and my opinions should be taken for what they are: the spouting of some random guy on the internet.

Okay, here's an update. I stumbled upon a five-part series about improving posture that seems reasonable to me. It's fairly technical and requires you to diagnose your own postural defects. I think we'd need Mr. Kurz to weigh in on whether the authors are correct with all this. It sounds plausible to me, but so do lots of things that turn out to be wrong.
